GENERAL

International Quality Registrars (IQR) Corporation has defined the Registration Service Fees as follows:

Application Fee Review of quality manual and all preliminary administration.	Contact Our Office.
Pre-Assessment (optional) Preliminary visit performed to review quality system and documentation prior to Registration assessment	Contact Our Office.
Registration Assessment Complete on-site assessment for conformance and effectiveness of quality system to the applicable ISO 9000 standard	Contact Our Office.
Surveillance Assessment Performed once every six (6) months or yearly to verify maintenance of quality system.	Contact Our Office.
Corrective Action Time spent by auditor for additional corrective action on-site or as follow up verification.	Contact Our Office.

Note(s):

1. Actual number of auditors and auditor days required to perform assessments will be based on:

Size of company	Technical requirements
Facilities & locations	Documentation complexity
Scope of products and services	Corrective action requirements

2. Registration provided by IQR is valid for a period of three (3) years from date of issue provided all requirements are met during surveillance assessments. Certificate is based on results from the six month surveillance or a reassessment for the annual surveillance.

3. A total of five (5) surveillance assessments are performed during valid period of Registration.
